Fatal error in admin interface

Hey there,

WP version: 4.6.12

Installed the plugin but cannot configure it via admin settings. The page throws a fatal error:

Plugin is shown in both WP Panel and Menu. Feedback page is also visible (/wp-admin/edit.php?post_type=helpful_feedback).
