Fatal error – media.php file has been changed

Several weeks ago my website – which has operated without errors for several years – crashed with the error “PHP Fatal error: Uncaught Error: Call to undefined function wp_get_additional_image_sizes() in /usr/www/users/jcseao/wp-includes/media.php:64” on loading.
I’ve spent several weeks trying to debug this, and I’ve read many comments on support forums.
I discovered that by simply replacing the media.php file with a “fresh” copy from the new WP installation “solves” the problem, for varying periods of time (a few hours to several days)…. and then it crashes again with the same fatal error.
I suspect that my site is being hacked – is this a valid assumption, or is there something else that might explain this. If it is a hacker, how do I protect my site?
