Favorite membership application form plugin?

I’m helping a client to change a PDF membership application form to an online form capable which members can fill out and submit on the client’s WP site. The form currently asks for the following info:
Names
Mailing Address
Phone
Email
Credit Card Payment info
Billing Address
Agreement

Any plugin out there that would migrate membership data into an Excel spreadsheet? Please help.
