You can easily customize form, check out the FAQ (Example of default form).

As for the captcha, based on my experience, there’s no need for it (I have not seen spam submissions with the plugin in two years).

Ok, no need for captcha, but how would I make a form without the Title and Description fields? (see screenshot: https://snag.gy/5lR0Y.jpg)
