Feedback form from registered users

Hi!
I want to get feedback from registered users only using Jetpack form.
How can I add their “display name” to e-mail/feedback form results, as they are allready loged in? I do not want to ask them once again fill their name/e-mail address.
Also — can I use the same form in multiple pages and automaticaly collect page address or title when they are submitting form?

So, basically, this is a question about collecting allready existing data.
User just fills their feedback, but page name/address, username and email is added automaticaly?
