Field data lost on resubmission

Hello.

I have a form: https://gethotseatapp.com/how-can-we-help/general-inquiriespress-inquiries/

If I test leaving a required form field empty and submit I get the proper error messaging.

However, if I fill in the missing field and hit submit I get returned to the form with my previously filled in data missing and required field error messages in its place. The form dumps my previously entered data.
