Fields: Image Attachment not working properly

    I created a query with:

    Template Style: unformatted
    Row Style: fields
    Posts Per Page: 3
    Fields: Post Title, Image Attachment
    Post Types: Portfolio

    For some reason the output isn’t showing all the Image Attachments. For example (the preview shows):

    – Title 1 (no image)
    – Title 2 + Featured Image
    – Title 3 (no image)

    All the porfolio items require a Title and Featured Image, so it’s nog missing. Any suggestions?
