Fields not showing in email

  • I set up a form including; name, email, phone, comments.

    When I get the return email, it gives me name, email, subject and comments.
    I can’t get the phone field to show in the return email, and I don’t know how/why subject does show?
    The form looks like:
    <label> Your Name (required)
    [text* your-name] </label>

    <label> Your Email (required)
    [email* your-email] </label>

    <label> Your Phone Number
    [tel tel-number] </label>

    <label> Tell Us How You Would Like To Participate
    [textarea your-message] </label>

    [submit “Send”]
