File Gallery Display Not Showing All Files

I’m attaching files on a page and having a gallery display said files (PDFs)

Problem is it isn’t showing all the files. It’s showing duplicates. For example:

I have Week 1 and Week 2 as two separate files. Depending on the sort order it will either show Week 1 file twice or Week 2 file twice.

Adding more files (Week 3 and so on) Ends up with a similar duplication. It shows Week 3 twice and Week 2 once.

Basically it’s not showing all the files as it should.
