File Upload placeholder show non-existent file

Fantastic Plugin!

I’m working on a site that is utilizing the File Upload Advanced meta box. The issue I am having is that when no file is uploaded, the box displays a single document icon with the post’s title as the name of the document.

Is this the default placeholder? In my case I think it could be confusing for the client. I’d rather is just not be there if no file has been uploaded.
