[Plugin: W3 Total Cache] Files not being served through CDN (mirror)

There are a few of my theme files and plugin files that are not either being served through or uploaded to my CDN, which is VPS.net’s Level3 CDN. Most other files work well, except for the ones listed below:

Any pointers or ideas as to why these are not on my CDN?

NOTE: the minified CSS files names have changed (dynamic everytime cache clears?)
