finding list associated with contact form drop down

I am trying to edit the options in a contact form on a website I did not build. The contact form is part of a private lead generating plugin that is still installed. I have gone through every possible file (I think) to try and find where the drop down options originate but I cannot find a place to edit them. I have found the js and php files for the form but it only refers to the options (medical procedure inquiries, labeled #uProc) but I do not understand where it is pulling the list from. In one spot it appears that it pulls from procedures.txt (can data be pulled from a text document?) but I have edited all occurrences of procedures.txt and it does not change it on the form. Is it possible that this information is stored with a third party? url is https://www.docmallory.com

I would prefer to not rebuild the form using a different plugin – it seems to still be functioning fine including backing up and storing all the lead form submissions.
