Fits to my requirements

I needed a plugin for a form with conditional logic, confirmation page with price, automatically sent mail to user and admin with a pdf invioce, customization of the pdf, show the form anywhere on the page, generation of participant list in frontend. For some requirements I used additional plugins for Gravity.
