Flag not showing in form

Hello, I have a couple of forms where I used your “flag” plugin but it doesn’t works well. In my website I have a contact form selector and when the user clicks any one of them, the form doesn’t load the flag next to the “telephone” input (and the field doesn’t event autocompletes the country dial code).

Do you know how can I fix this?
