Flat rate shipping quantity formula

4 + ( 4 * [qty] )

So with this formula if someone orders one item, it will be 8, but then each additional item will add 4?

Yes:

1 item: cost = 4 + 4 * 1 = 8
2 items: cost = 4 + 4 * 2 = 12
3 items: cost = 4 + 4 * 3 = 16
4 items: cost = 4 + 4 * 4 = 20
