This is more feedback than an issue needing troubleshot. In the event it’s not already in the roadmap, it’d be awesome if the plugin detected if/when an install is running 3.5+ and wasn’t an upgraded MU site. That way it could conform to the new location for MU files instead of forcing the blogs.dir folder structure.
