Forgot Password page – customise

HI there,
The forgot/reset password page rendered by the following short code [wpmem_form password] is asking for: username and email address.
Currently we have it set up so subscribers can use either their customer code or email address as their username – so we’d like the forgot password to just ask for their username which could be code or email but not both.
How can I change this to how it used to work as until recently you just needed to enter one input to get the password reset email sent – but now both fields are needed.
