Form and image upload

Hey Folks,

I’m looking for a bit of help.

I want to create a page where I can have customers fill in a form and upload some images. A basic form won’t be goo enough for this reason.

I’d like to enter various boxes where they can input text or select text from a drop down. And also a facility for them to upload images to send to me.

Ideally I’d like them to be able to save progress on the form too as it can be quite long winded, and they can complete later on down the line.

If there is a site that can do this, or any worpress plugins that may allow it on my own site, could anyone please let me know?
