Form appears entirely different when viewed from the default browser of my phone

It works fine everywhere, except the default browser of my mobile phone. This is a caching issue, I guess. Because it didn’t cause any issue when I viewed the same page from another device (same model). So, sharing the link with you wont be of any help because, you are viewing this webpage for the first time, and hence no caching issues would occur.

As it sounds, the issue is actually related to your phone in particular — caching is happening on your client device, not on the server. Probably forcing a cache refresh on your device could help? As much as we’d like to suggest a way out of this, we can’t think of anything in particular. Maybe you could try browsing from an incognito tab on your phone browser? That often helps with caching issues.

viewing the webpage in incognito mode didn’t change anything. The green “send” button is blue now, forms are too wide and goes out of the screen, white title is black now, “success notice” colour is different. Everything is messed up.

I’m a bit out of ammo on this one — it definitely sounds like some local caching, but we can’t really offer support for that. Did you try clearing your local cache in any way?
