Form Breaks after a few days – EVERY WEEK

    My contact form stops working (i.e. it does not capture nor submit the results) after a few days of adding the form to the page. The form worked great for months, but recently it just stops working.

    If I delete the form from the page and re-add it, it works for a few days and then breaks again.

    Any ideas? Has anyone had a similar issue? Could malware cause this issue?
