Form broken

    When I submit my form there are two issues:

    1. Half of the time I don’t get an email. The submit button turns green and says submitted, but I never get the email (I checked spam and promotions and couldn’t find anything)
    2. When I do get the email, it only has the responses to the first 4 questions. It looks like this, and instead of responses it just has numbers (1-11) until the end of the email. Here are my settings (email removed for privacy reasons, but every other field is the same).

    I fixed the second issue, but the first issue remains.

    For your First issue it could be something with your SMTP configuration of the WordPress. If the plugin gave the information that the mail is sent, it could be also an issue with the hosting.
    You need to check this.
