Form builder recommendations please.

I need to create a quote form that users can answer a few questions and get a personalised quote.
I’ve been able to do it with Ninja Forms, but the styling has proved to be a problem and the tooltips don’t work.
It doesn’t have to be a plugin as long as it can be embedded.
