Form Completion Checklist

Hope you are doing fine.

This could be implemented using Forminator and some custom code using Advance Custom Fields. Keep in mind we may not provide the complete code to implement this functionality, but can direct you to online resources where you can get further information.

1. Capturing user information in the forms.
   You can capture the user information including a hidden field and applying the username as the default value. In this way, you can keep track of the user that submitted the info. Please refer to this url for further info:

2. Create a Advance Custom Field and attach it to the user profile. The following thread can provide very useful info:

3. Then you can use custom SQL queries to gather more info from the tables that hold the form submissions and look if the ones for the current user are listed. Please refer to this article:

   The form data is stored in the following tables, you can review their contents to understand a little more how the records are saved:
   wp_frmt_form_entry
   wp_frmt_form_entry_meta
