Form disappears

    On the website samenwerkt.com i want to add a form with a lot products and questions. But when i add the shortcode into the wordpres page nothing happend. When i use another plugin everything works fine. Any suggestions?

    So the same thing happens to me. What’s the answer?

    So the information is not retained when I click save. It seems to be the same thing that’s happening to this person. Every time I save on your template, then go back to it, my information is not there.

    Please help.
