Form does not appear

Hello,

I am trying to get your plugin to work for hourly bookings between 9am-9pm on a daily basis for free. I believe I have everything set up correctly, but cannot get the form to show after an hour is selected on the front end. I have double-checked that the “View-Only” option is disabled. Could you look into this for me please?
