Form does not appear (remain black)

Since the mentioned page URL returned a 404 error, I couldn’t take a closer look at the issue. Can you please share a page URL where you have added the shortcode?

Please also take an export of the form and share using Google Drive or DropBox which will help us to debug the issue further.
