Form doesent work on simple product

    I am using uncode theme and woocommerce. The form doesn’t work on simple page product but it works fine on variable product. Could you help me to solve this issue?

    i have the same problem with uncode theme, works fine on variable product but form not showing in simple product, here the link of my website:
