Form fails for custom woocommerce account pages

Ahoy,
In line 400 you set the form action attribute with the_permalink()
Unfortunately for woo account pages this resolves too:
https://courses.21gratitudes.com/account/
Instead of:
https://courses.21gratitudes.com/account/example-pageAs a result, the form fails to upload.
