Form Field Created By Customers

  • Hopefully someone can help me.
    I cant seem to find what I’m looking for, but maybe I’m not using the correct search terms.
    What I need to do is have a contact form on my site. But I need to have name, details etc. but with the option to add details for a second or third person.
    Like a little plus or + sign that when clicked adds the fields to add the same details for another person.
    Anybody have any idea is a plugin like that exists?
