Form Fields do not Function

    Unfortunately the feature for the form fields and form submissions does not work.
    Therefore reviewers cannot add their name, text review, star rating or any other similar functionality.

    They can only upload a video, but as the site owner we have no context as the fields we require a reviewer to complete are not stored/saved.

    Happy to update the review if this is resolved.
