Form is not creating lead. Am I doing something wrong?

I set up my salesforce per the installation instructions but my leads are not being updated in salesforce. I noticed some of the form fields have a check box to enable field mapping. Is this required? My id and security token are being accepted and I setup the feeds. Any help would be greatly appreciated.
