Form not displayed

Hi Grzegorz,
This is a great plugin, I have been looking for this for days.
I installed this with ACF Version 4.4.1 and Forms Actions Version 1.2.2 on my WordPress Version 4.2.1 but the form doesn’t get displayed on the defined page.
I was trying to create a post to a custom post type and thought that was the problem and changed it to the default post but still the problem exists.I also see an error on page:
GET https://localhost/snnl/wp-content/plugins/acf-frontend-display/js/acf-frontend-ajax?ver=4.2.1 404 (Not Found)Please let me know how I can fix this!!
