Form not displaying

Just noticed that one of my forms on my site isn’t working. I have two on the site one works fine and the other isn’t displaying it just shows /*/*]]>*/

They both are referenced by shortcodes but even when i preview i get the same result.

I edited another form that was not in use that worked at first but after i edited it displayed the same issue.

Any ideas anyone i’m on Version 3.1.8
