Form Not Displaying Correctly

Hi. I have created a MailChimp signup form which looks fine on any WordPress page. But when I add the code to the plugin, it doesn’t look right on the page. Some styling doesn’t get applied (e.g. background color). Not sure what’s causing this, any advice? Thanks.
