Form not displaying on mobile phones.

I was able to replicate the same issue for both desktop and mobile.

According to the developer console, the issue seems to be related with Litespeed plugin.

Please try enabling Load form using AJAX and Prevent page caching on form pages options under Edit Form->Behavior->Rendering and purge your caches on Litespeed.

If this does not help try deactivating Litespeed, if you can see the form then check asset optimization settings for CSS and JS on Litespeed which seems to be causing the issue.

Sorry, to hear the issue is still the same. Just to be sure, were you able to check and confirm if the issue was the same even with the Litespeed plugin temporarily disabled?

It seems like the page URL you have shared no longer loads and is getting redirected to the homepage, neither I could find Forminator enabled, so we couldn’t check further.

You could also use a staging site to test the anomalies noticed as it would help in ruling out if the issue is specific to the plugin or not.
