Form not displaying on Product Detail page

    Hi, I set up a form using the plugin, and checked the form I created in the edit product section. But the form is not displaying on the product page. Is there something I am doing wrong? thank you

    Are you able to see the add to cart button on the product page? Please ensure you have set price for product.
