Form not loading

Hello,
My form is not loading and seems to prevent the page from completing its load:
https://careers.abcstores.com/apply-page-1/I’ve recently updated to the most recent WordPress and updated all my plugins. I’m currently using Contact Form 7 Multi-Step Forms v 2.0.2.
When I turn off the Contact Form 7 Phone Module, everything seems to work correctly.
