Form Not Loading

Hello,

I’ve been working for days on a condition-based form. Today I published the form to start testing it on our template. While the preview works in the plugin dashboard, where you create it… when I drop the shortcode on our pages, even a brand new page, and clear 100% cache on the site, it doesn’t work.

I read your team member’s advice to use the following custom CSS, but it makes no difference.

.forminator-custom-form {
display: block !important;
}

I have published a test page in case you need to check it out, the only thing that is added to the page when the shortcode is added is the following… which does not contain the actual form fields.
