Form not loading-just spinning on mobile

I checked your form here — https://youthexperiencesponsorship.org/donations/donation-form/ both on mobile (Chrome developer tools and my own mobile phone) and desktop, but the form is working fine for me.

I also didn’t encounter the JavaScript error that you mentioned.
