form not showing on WooCommerce product

  • Hello,

    Thank you so much for this plugin, I am testing the 2.9.8 free version in order to see if I will buy the paid version to offer multiple services.

    In this test, I sell phone audits and I want to show the form that I created in order to know if my client prefers that I call him on Whatsapp or Zoom and what he needs help with.

    Unfortunately, this form is not showing when I make it a WooCommerce product. Is there an option to check that I have missed?

    Quick note : For now, I make do with free extras but I would like to offer the possibility for the client to write details.
