Can you disable all your other plugins except this one. Carry out another test using the text widget. If that does not work then we can safely say that the problem is with your theme. If that is the case you will have to get in contact with the theme developers.

I too am having a problem with the form displaying the labels. I see the radio buttons and text area, but no labels. What am I doing wrong

@deb, I think your issue is with your theme. Can you test one of WordPress default themes like Twenty Fourteen or Fifteen?
