Form Not Submitting After Latest Update

At this point, we have had Multi-Step Plugin support, Image Upload Plugin support, and our internal programming team look at this and fix some issues, but the main issue persists. This is our main form and it has been down for 10 days troubleshooting this issue. We have exhausted all other avenues at this point and need help. (Note: we have run more than 500 submissions off this form and it worked great until the latest update). Please let us know what you need to proceed.

PS: we have fully cleared the Siteground caches, the browser caches, the local machine caches — all the caches we are aware of and still not sending the form.
