Form not working with url, file, acceptance fields

I tested your form on your site. I think this is related to either Contact Form 7 or your page permalinks.

Without changing your form or settings at all can you disable the PayPal addon and see if it submits correctly.

I have remove the paypal plugin and the form submits okay.

If I enable the PayPal plugin the problem reoccurs again.

I have tried all the perma links setting with no luck!

If we can get this problem solved I would like to buy the pro version as it has some options we would like to use.

Problem fixed, was related to using “name” as the name of a form item:
