form numbers showing

Hi There,

Recently I noticed that the form ID number was showing up on my pages. See on here:
https://www.jeanettekwok.com/contact-us/ and also the bottom of this page:
https://www.jeanettekwok.com/seo/

The first one is using the shortcode [customcontact form=3] and the second one is using
[customcontact form=5] and you can see that the “3” and “5” are showing up respectively on each page very randomly.

How do I resolve this?
