Form Plugin

Hi folks,

Hope you are well.

I have a 10 page document that needs turning in to an online version. It has blank (text) boxes and tick boxes. I want it 1 page at a time and then click next. The site will be built on WordPress and this would need to be a plugin that can be updated. Anyone know of a plugin for this or if not could anyone give me a quote to build. Happy for you to resell this afterwards.
