You simply should insert three media fields for the images, and assign to each field the special class name: column3, through the field attribute: “Add Css Layout Keywords”

You can get additional information in the answer to the question: “Q: Is there a way to format the form in a table structure (various fields in the same line) ?” in the FAQ page of the plugin (https://www.ads-software.com/plugins/calculated-fields-form/faq/)

Found a glitch it wont line up the first column tried checkbox and radio buttons. Its okay for images.

I’ve tried to visit your webpage but your server is responding with a: 404 Error, page not found.

If you are using the class name column3, you should apply it to three consecutive fields.
