Form submission = 404; deleting form elements does not work

New install of CFM on a WP 3.5.1 install (several other active plugs but nothing that’s obviously interfering with CFM).

Creating and displaying a form on page works fine, however when attempting to delete form elements in the editor, they remain displayed in the list. Refreshing the page to update the shows “0” in the right hand box and a full reload of the page is required.

Forms render properly on pages, but when submitting a 404 error occurs. I’m attempting to use two forms on two Pages.
