    Have you tried rebuilding Permalinks?

    I changed themes, but that’s it. What do you mean by rebuilding permalinks? In the form or for the page?

    Changing themes could have that effect. Some themes have very little form styling built in…

    Nevermind about rebuilding permalinks — you gave me a bad URL… which I though was a permalinks issue.

    Since I don’t know what the form used to look like, you’ll need to be more specific… since it looks fine to my eyes…
