Form too wide on narrow screens

    I just noticed a bug where the form is too wide on narrow screens (e.g. mobile phones). The form displays correctly on wide screens and a narrower version of the form displays correctly on medium sized screens.

    I reproduced this bug in version 3.0 of the plugin in versions 4.9.11 and 5.3.0 of WordPress.

    Since the form still works, I do not consider this a very urgent problem, but please let me know if you disagree. I will probably try and fix this in the next version of the plugin.
