Form with data plugin question

Hi, Im building a very small website, where users will register and submit their requests in a form.

I need such a form plugin, which should list all the submissions what the user has done .

For example, Im a user.

I register and submit my requests at the website form.

Then there should be a menu, MY REQUESTS.
Where I can simply go and see, what I had requested in details and in what data for my request records.

Any plugin, which you know, can do this job for me ?
